Primary Care Physician
The Primary Care Physician will provide patient centered primary care medical services to all Veterans in collaboration with a dedicated RN and LPN. Duties include but aren't limited to: Develop relationships with patients over time. Manage chronic conditions, evaluate and treat current medical conditions, and screen for onset of new health related conditions. Collaborate with other primary care teams and providers when needed. The primary care team practices and coordinates patients' care with support of Pharm D, social worker, dietician, integrated mental health professionals, and a broad range of medical, surgical, and mental health specialists either on site or by telemetry. May perform occasional minor office procedures common to general internal medicine and family practice skills.
