Physician (Primary Care)
Primary Care physicians provide primary care services to established panels, adult through geriatric, Veteran patients and functions as a member of a patient aligned care team (PACT) consisting of an RN Care Manager, a clinical associate (LPN or Health Tech/Medical Assistant), and a clerical associate (Medical Support Assistant). The PACT model provides comprehensive interdisciplinary care in coordination with extended support (e.g. pharmacist, social worker, dietician, psychologist, etc.). To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement.
