The selected candidate will provide comprehensive, developmentally appropriate care for adolescents and young adults with chronic, complex or special healthcare needs. The role focuses on guiding patients and families through the transition from pediatric to adult healthcare, promoting independence, self-management and continuity of care across health systems. Key responsibilities: Provide direct medical care to youth and young adults with chronic or complex medical conditions Conduct transition readiness assessments and develop individualized transition care plans in collaboration with patients, families, and prov
